Care Partners Project
This video describes the Care Partners: Bridging Families, Clinics, and Communities to Advance Late-Life Depression Care project, which brought together primary care clinics and community-based organizations (CBOs) to treat depression in older adults in California. The following organizations featured in this video are: Neighborhood Healthcare and their CBO partner Interfaith Community Services, Kaiser Permanente Southern California and their CBO partner Healthy African American Families II, and Depression and Bipolar Support Alliance and their clinic partner Facey Medical Group.
